Thank you for choosing Dr. Grant A. Fairbanks for your medical care. Below we have listed some of our policies to help familiarize you with our office. If you don’t find what you are looking for here, be sure to contact us. We’ll be happy to help you. Our friendly staff is eager to help you set up a consultation and answer any questions that you may have.

Consultations
& Cancellation Policy:
Dr. Fairbanks sees patients by appointment either online or in person. Our Consultation Fee is a non-refundable, upfront payment of $150. To schedule an appointment we require a valid credit card and an immediate $150 payment. Please understand that our Consultation Fees are non-refundable as our appointment times fill up fast and are very limited. If you must reschedule, we respectfully request 48 hours notice.
Those who are more than 15 minutes late, no-show, cancel late, or need to reschedule without 48 hours notice, will still be charged the non-refundable $150 Consultation Fee for that day's appointment time and will be required to pay another $150 Consultation Fee upfront in order to schedule a new appointment time.

A Consultation with Dr. Fairbanks is truly a unique experience. Plan for him to spend a full hour with you. Whatever procedure(s) you're interested in he'll take the time to answer all of your questions and in this one consultation, he will ensure you know exactly what to expect from start to finish.
Dr. Fairbanks often draws out animations of the procedure process. He also shares his recommendations for your specific case plus his vision for the end result. Once finished, people feel fully prepared, excited, and confident about proceeding with surgery and usually book their appointment before leaving.

How to Cancel Your Appointment:
Dr. Fairbank's schedule is booked out months in advance and we always have a long waitlist. To accommodate all that want to see him, the team at Fairbanks Art Of Plastic Surgery respectfully requests 48 hours notice if you must cancel or reschedule your appointment, by CALLING or TEXTING our Office @: (801) 951-8099 Monday - Friday between 9 am - 5 pm.
If necessary, you may leave a detailed voicemail or send us a text message with your name, original appointment date and time, and the reason for needing to cancel or reschedule. We will return your call as soon as possible.
Late Cancellations / No-Shows:
We understand that sometimes the truly unexpected can occur, requiring the need for schedule adjustments. We will do our very best to reschedule you for another time that is convenient for you.

Requirements
For Prescription Refills
When needing a prescription refill, please contact our office 48 hours in advance. As a general policy, we do not refill prescriptions after hours or on the weekends. When you call our office to request the refill, please have the following information available:
1. Name of your medication
2. Dosage of your medication
3. Pharmacy name and phone number
4. A number where we can reach you with questions
